Proposal
Guidelines
Please
answer the following questions on a separate piece of paper. Your
completed proposal should be no more than 3 pages, not including
supporting documentation and budget information.
Please
arrange your proposal in the following order and answer the following
questions:
A.
Agency/Organization Background
1.
Briefly description of your organizationšs history, mission and
goals.
2.
Relationship with other organizations providing similar services.
3.
What you regard as appropriate diversity for your organizationšs
board, staff and served constituency. How
and over what period of time you intend to achieve your objectives,
if you have not already done so.
B.
Funding Request
1.
Briefly describe the need(s) that this project will address in the
community?
2. Briefly describe the program/project to be funded and its anticipated
significance both for the organization and the community.
3.
What are your program objectives?
4.
Who was involved in planning and how will they benefit?
5.
What strategies will you use to organize and implement your project/program?
6.
What is your timetable and anticipated length of the program?
7.
How will you involve other organizations and individuals in the
design and delivery of the project? What organizations or individuals
will you recruit to participate?
8.
What other sources of funding does this project/program have? Describe
additional fundraising efforts. How will the program be sustained?
D.
Evaluation
1.
What results do you expect to achieve from your project?
2.
How will you measure your results/outcomes?
3.
How will you communicate this information to your funders?
E. Proposed Project Budget (no
more than one page)
Please
provide a line item budget for the proposed project including. Please
explain line item requests in detail (ex. Uniforms, 10 x $50 = $500
or Field Trips, 5 x $150/trip = $750). Please list other income
sources and funders and the support they are providing.
